
Feeling off-balance while walking can be unsettling, especially when you are not sure why it is happening. Feeling off-balance means having difficulty maintaining a stable body position while you move. You might feel as though you are swaying, staggering, stumbling, drifting, or losing your footing. You may also feel like you are going to fall even when you have not actually fallen.
Not everyone with a balance problem experiences dizziness or vertigo. Some people feel perfectly normal while sitting or standing still but become unsteady once they start walking. This is often described as a problem with dynamic balance, which is the ability to maintain stability while moving. Certain situations can make the problem more noticeable, such as:
Walking while turning your head
Walking in a dark room or poorly lit area
Bending down and standing back up
Walking on uneven surfaces

Trying to do another task while walking

1. Inner Ear or Vestibular Problems
Your inner ear contains the vestibular system, which helps your brain understand where your head is in space and whether you are moving. Problems affecting this system can cause dizziness, vertigo, or unsteadiness.
One common example is benign paroxysmal positional vertigo (BPPV). It occurs when tiny calcium crystals inside the inner ear move out of their normal position. BPPV commonly causes brief episodes of spinning when you turn in bed, look up, or change your head position.
Other vestibular conditions can also affect walking. Vestibular neuritis, for example, can cause severe dizziness, nausea, and difficulty walking, with symptoms sometimes lasting several days. Meniere's disease and vestibular migraine can also cause dizziness and balance problems, although they have different patterns and associated symptoms.
2. Muscle Weakness and Deconditioning
Walking requires coordinated strength throughout your legs, hips, trunk, and feet. When these muscles become weak, maintaining stability and correcting a sudden loss of balance can become harder.
This can happen after an injury, illness, prolonged inactivity, or simply from spending too much time sitting. Reduced physical activity can contribute to muscle weakness, which can then make balance problems worse.
3. Nerve Problems
Your brain needs information from nerves in your feet and legs to know where your body is positioned. This sense is called proprioception.
Conditions that damage peripheral nerves can interfere with this information. Diabetes, for example, can affect sensation in the feet and make it harder to know exactly where your feet are contacting the ground.
You may notice more difficulty walking on uneven surfaces or in the dark because you cannot rely as heavily on your vision to compensate for reduced sensation.
4. Vision Problems
Your eyes provide important information about your surroundings and your body's position. Blurred or impaired vision can make it harder to judge obstacles, surfaces, distances, and movement.
Balance can become particularly challenging when visual information is reduced, such as when walking at night or in a dimly lit room.
5. Joint Stiffness or Pain
Your joints also contribute to balance by helping you move and adjust your body quickly. Stiff ankles, knees, hips, or other joints can limit your ability to respond when your balance is disturbed.
Arthritis and other musculoskeletal problems can therefore contribute to an unsteady walking pattern.
6. Low Blood Pressure, Dehydration, or Circulation Problems
Sometimes the sensation is less about balance itself and more about feeling faint.
A sudden drop in blood pressure when you stand up, known as orthostatic hypotension, can cause lightheadedness or faintness. Dehydration and certain cardiovascular conditions can also reduce blood flow and contribute to these symptoms.
If your main symptom is feeling as though you might pass out rather than feeling physically unsteady, it is important to discuss it with your physician.
7. Medications
Some medications can cause dizziness, lightheadedness, or unsteadiness as a side effect. This can be particularly important if your symptoms began after starting a medication or changing its dose.
Do not stop a prescribed medication on your own. Instead, ask your doctor or pharmacist whether any of your medications could be contributing to the problem.
8. Neurological Conditions
Certain neurological conditions can affect coordination, strength, sensation, or the brain's ability to process balance information. Examples include Parkinson's disease, multiple sclerosis, stroke, and some spinal conditions.
These conditions usually require medical evaluation rather than treating the balance symptom alone.
Occasional unsteadiness may have an obvious explanation, but recurring or worsening balance problems deserve attention, particularly if you have fallen or are changing your normal activities because you are afraid of falling.
A physical therapist can assess strength, movement, coordination, visual tracking, balance, and walking ability and ask about when your symptoms occur. Depending on the findings, you may also need evaluation from a physician, eye doctor, or ear specialist.
Seek urgent medical attention for sudden balance problems accompanied by symptoms such as new weakness or numbness, trouble speaking, severe headache, fainting, chest pain, or sudden vision changes, because these can signal a serious medical problem.
Finding the cause of an unsteady gait starts with understanding when and how your symptoms happen. A physical therapist may ask whether you feel spinning, lightheadedness, weakness, or simply unstable. They may also ask about falls, medications, vision or hearing changes, previous injuries, and activities that make your symptoms worse. The physical examination may include:
Walking and gait assessment
Balance testing while standing and moving
Leg and core strength testing
Joint mobility
Coordination
Visual tracking
Sensation and body-position awareness
Vestibular or inner-ear screening
Functional movements such as turning, reaching, or getting up from a chair

Weakness in the legs, hips, and trunk can make it harder to maintain or recover your balance. Strength exercises can help you build the force and control needed for walking, stairs, transfers, and other daily activities.
Research supports combining strength and balance exercises for people at risk of falls. A Cochrane review involving 108 randomized controlled trials and 23,407 participants found that exercise reduced the rate of falls by about 23% in older adults living in the community. Programs combining balance and functional training with resistance exercise were particularly useful.
Vestibular Rehabilitation
If your imbalance is related to the inner ear or vestibular system, treatment may include vestibular rehabilitation. Depending on the condition, exercises can address gaze stability, motion sensitivity, balance, and coordination between your eyes, head, and body.
Physical therapists with vestibular training commonly treat conditions such as BPPV and vestibular neuritis and can work on both dizziness and gait problems.
Mobility and Flexibility Exercises
Stiff joints or tight muscles can change the way you walk and make it harder to react to a loss of balance. Treatment may include exercises to restore joint movement, flexibility, posture, and functional mobility.

If you are already falling or feel afraid of falling, treatment may also focus on making everyday movement safer. Your therapist may assess how you walk, how you respond to a sudden loss of balance, your footwear, and potential hazards around your home.
The safest exercises depend on the reason you are unsteady. If you have frequent falls, significant dizziness, or an unexplained balance problem, it is better to be evaluated before attempting challenging balance exercises on your own. In general, staying physically active can help maintain strength and mobility. Your physical therapist may prescribe exercises involving:
Leg and core strengthening
Balance practice
Walking and gait exercises
Posture and flexibility work
Eye-tracking or vestibular exercises when appropriate
Functional activities such as stairs or getting up from a chair
Balance exercises should be progressed gradually. For example, a therapist may move from supported to unsupported standing, then introduce narrower foot positions, head movements, different surfaces, obstacles, or other real-life challenges.
Evidence also suggests that regular exercise can reduce falls. In the Cochrane review mentioned above, balance and functional exercise programs reduced the rate of falls by approximately 24% compared with inactive controls.
That does not mean a particular exercise will work for everyone. The important point is that the right exercise should match the reason you are losing your balance.
Most balance problems are not emergencies, but sudden or severe symptoms should not be ignored.
